Sus antecedentes:

  • La Iniciativa de Investigación de Enfermedades Neurodegenerativas de Ontario (ONDRI) es un estudio de cohorte prospectivo en Ontario, Canadá.
  • Inscribió a personas con enfermedad de Alzheimer y/o deterioro cognitivo leve (ADMCI), enfermedad de Parkinson (PD), esclerosis lateral amiotrófica (ELA), demencia frontotemporal (FTD) y enfermedad cerebrovascular (CVD).
  • El estudio tuvo como objetivo analizar la utilización a largo plazo del sistema de salud, las transiciones a hogares de ancianos y la mortalidad en estas condiciones neurodegenerativas.

Objetivo del estudio:

  • Examinar las diferencias a largo plazo en el uso del sistema de salud entre pacientes con diversas patologías neurodegenerativas.
  • Investigar los patrones de transición a la atención en hogares de ancianos en diferentes enfermedades neurodegenerativas.
  • Comparar las tasas de mortalidad asociadas con distintas condiciones neurodegenerativas.

Principales métodos:

  • Se vincularon datos administrativos de salud de 478 participantes de ONDRI (inscritos entre 2014 y 2017) con evaluaciones clínicas.
  • Se realizó un seguimiento de los participantes hasta el 31 de marzo de 2024, la muerte o la pérdida de elegibilidad para el seguro de salud.
  • Se calcularon las tasas de uso de servicios de salud, transiciones a hogares de ancianos y mortalidad por cada 100 personas-año, estratificadas por patología neurodegenerativa.

Principales resultados:

  • La esclerosis lateral amiotrófica (ELA) presentó la tasa de mortalidad más alta (35,3 por 100 personas-año), mientras que la enfermedad cerebrovascular (CVD) tuvo la más baja (2,4 por 100 personas-año).
  • Los pacientes con ELA mostraron el mayor uso de atención domiciliaria, los pacientes con enfermedad de Parkinson (PD) tuvieron la mayor cantidad de visitas de emergencia relacionadas con caídas y los pacientes con demencia frontotemporal (FTD) tuvieron la tasa más alta de transición a hogares de ancianos.
  • Las personas con enfermedad de Alzheimer y/o deterioro cognitivo leve (ADMCI) fueron mayores en promedio que las personas con ELA.

Conclusiones:

  • Las patologías neurodegenerativas requieren servicios de salud personalizados con diferente intensidad.
  • La vinculación de datos administrativos de salud con cohortes clínicas permite un monitoreo rentable y a largo plazo de los hitos de los servicios de salud.
  • Los hallazgos resaltan las diversas trayectorias de atención médica y las necesidades de recursos en diferentes enfermedades neurodegenerativas.
