Sus antecedentes:

  • Se realizó una evaluación de procesos de la intervención RecoverED, un programa de rehabilitación domiciliaria multidisciplinaria para el delirium en adultos mayores en atención postaguda. El estudio utilizó un Modelo Conceptual Modificado para la Fidelidad de Implementación para evaluar la viabilidad y aceptabilidad de la intervención.

Objetivo del estudio:

  • Evaluar la implementación y aceptabilidad de la intervención RecoverED en un ensayo de viabilidad mult sitiopara evaluar la adherencia al contenido, la dosis y la cobertura de la intervención, e identificar los factores moderadores que influyen en la implementación.

Principales métodos:

  • Diseño de métodos mixtos que involucró a adultos mayores con delirium, sus cuidadores y profesionales de la salud (PS) en seis hospitales del NHS del Reino Unido. La recopilación de datos incluyó entrevistas en profundidad, grupos focales, documentación del ensayo y registros de capacitación/supervisión, y los hallazgos se triangularon.

Principales resultados:

  • Se reclutaron 19 pares de participantes-cuidadores, con 5 adultos mayores, 9 cuidadores y 8 profesionales de la salud (PS) entrevistados. La adherencia al contenido fue compleja debido a la naturaleza personalizada de la intervención. El apoyo psicosocial se administró en exceso, mientras que la rehabilitación física se administró en defecto. Las metas dirigidas por el participante fueron valoradas, lo que generó alta satisfacción y compromiso. La fidelidad de implementación al enfoque de administración fue alta, con comentarios positivos de los participantes sobre la calidad de la administración. Los PS desearon una capacitación más interactiva y la ubicación conjunta de los equipos se identificó como crucial para la coordinación.

Conclusiones:

  • La intervención RecoverED demostró ser aceptable entre los participantes. Los desafíos de reclutamiento exigen precaución al interpretar la aceptabilidad y la fidelidad a la dosis y la cobertura. Se logró una alta fidelidad de implementación al enfoque de administración y fue percibida positivamente.
