Sus antecedentes:

  • Un período reproductivo (PR) más largo, caracterizado por una menopausia de aparición más tardía, se asocia con una mejor función cognitiva y un menor riesgo de atrofia cerebral relacionada con la edad.
  • Comprender el impacto del PR en las capacidades cognitivas y la estructura cerebral en mujeres posmenopáusicas es crucial para las estrategias de salud pública.

Objetivo del estudio:

  • Investigar el rendimiento cognitivo y las diferencias en el volumen cerebral en mujeres posmenopáusicas en función de la duración de su período reproductivo.
  • Comparar las funciones cognitivas y los marcadores de neuroimagen entre mujeres con períodos reproductivos cortos y largos en una población urbana india.

Principales métodos:

  • Se reclutaron 583 mujeres posmenopáusicas (edad ≥45) del Estudio Longitudinal Tata de Envejecimiento.
  • Se evaluó a las participantes mediante exámenes clínicos, cognitivos, bioquímicos y neuroimagen por RM de 3T.
  • Se calculó el PR (edad de la menopausia menos edad de la menarquia) y se clasificó a las mujeres en grupos de PR corto (≤35 años) y largo (>35 años).

Principales resultados:

  • El grupo de PR corto presentó una educación significativamente menor, una edad más temprana en el primer parto y niveles más bajos de ácido fólico.
  • Los análisis ajustados revelaron que las mujeres con un PR corto tenían una peor función de atención (β = -0.417, p = 0.023).
  • El grupo de PR corto también mostró una reducción significativa en la sustancia gris total (β = -6464.403, p = 0.026), el volumen cortical (β = -4827.836, p = 0.044) y el volumen cortical del hemisferio derecho (β = -2971.033, p = 0.018).

Conclusiones:

  • Un período reproductivo más largo se asocia con una mejor atención, preservación de la sustancia gris y mayores volúmenes corticales en mujeres posmenopáusicas.
  • Estos hallazgos sugieren posibles efectos neuroprotectores de la exposición prolongada a los estrógenos, que podrían mitigar la atrofia cerebral relacionada con la edad.
  • Se justifica una mayor investigación para dilucidar los mecanismos subyacentes y las implicaciones para la salud pública de estas asociaciones.
