Sus antecedentes:

  • La enfermedad de las pequeñas arterias cerebrales (ECVSC) se caracteriza por desmielinización, y la actividad física está relacionada con un mayor contenido de mielina en adultos mayores.
  • Los mecanismos que conectan la actividad física y la salud de la mielina en la ECVSC no se comprenden bien.
  • Las lipoproteínas de alta densidad (HDL) desempeñan un papel en la remielinización cerebral y están influenciadas por la actividad física, lo que sugiere un papel mediador potencial en la relación actividad física-mielina.

Objetivo del estudio:

  • Investigar si los niveles de lípidos séricos, específicamente HDL, LDL y triglicéridos, moderan la asociación entre la actividad física y el contenido de mielina en personas mayores con ECVSC y deterioro cognitivo leve.

Principales métodos:

  • Estudio transversal que involucró a 81 adultos mayores con ECVSC y deterioro cognitivo leve.
  • Se utilizaron modelos de regresión múltiple para evaluar el efecto de moderación del HDL, LDL y triglicéridos en la relación entre la actividad física y el contenido de mielina.
  • Los análisis se ajustaron por edad, sexo, educación, IMC, colesterol total, volumen intracraneal y lesiones de sustancia blanca.

Principales resultados:

  • La actividad física se asoció positivamente con los niveles de HDL y el contenido de mielina en varias regiones cerebrales, incluyendo la corona radiada anterior, el genu del cuerpo calloso y el estrato sagital.
  • El HDL se asoció positivamente con el contenido de mielina en múltiples áreas cerebrales, como el cuerpo del cuerpo calloso y la corona radiada posterior.
  • Los niveles de HDL moderaron significativamente la asociación entre la actividad física y la mielina en el estrato sagital, con una mayor actividad física vinculada a una mayor mielina en individuos con HDL promedio o alto.

Conclusiones:

  • La actividad física puede contribuir a la salud de la mielina en personas con ECVSC, al menos en parte, a través de mecanismos que involucran el HDL.
  • Se requieren estudios longitudinales para establecer la causalidad y confirmar el papel del HDL en la mediación de los efectos de la actividad física sobre la mielina.
