Sus antecedentes:

  • La demencia a menudo se acompaña de comorbilidades que pueden empeorar el declive cognitivo.
  • Los países de ingresos bajos y medianos (PIM) soportan una carga significativa de demencia debido a recursos limitados y poblaciones que envejecen.
  • Los datos del mundo real de los PIM son cruciales para comprender las comorbilidades de la demencia.

Objetivo del estudio:

  • Identificar las comorbilidades clínicas asociadas con la demencia en un entorno de PIM.
  • Investigar cómo los clústeres de comorbilidades impactan el declive cognitivo longitudinal.

Principales métodos:

  • Análisis retrospectivo de 500 pacientes adultos de una clínica de memoria neurológica brasileña.
  • Clasificación de los participantes en deterioro cognitivo subjetivo (SCD), deterioro cognitivo leve (MCI) o demencia.
  • Regresiones logísticas para evaluar las asociaciones de comorbilidad-demencia y modelos de efectos mixtos para las trayectorias del MMSE.

Principales resultados:

  • La hipertensión y la baja educación fueron muy prevalentes.
  • El accidente cerebrovascular y el consumo de alcohol fueron predictores significativos de demencia.
  • Los clústeres de comorbilidad cerebrovascular, neurosensorial y de abuso de sustancias se vincularon con un declive cognitivo más rápido.

Conclusiones:

  • La multimorbilidad es común en los PIM, con cargas cerebrovasculares, neurosensoriales y de abuso de sustancias que contribuyen al riesgo y la progresión de la demencia.
  • La baja educación exacerba la vulnerabilidad al declive cognitivo.
  • Se necesitan intervenciones de atención primaria para controlar las comorbilidades y reducir la carga de demencia en entornos con recursos limitados.
