  • El impacto del NE en la reserva cognitiva puede diferir entre grupos raciales/étnicos y genotipos APOE, lo que requiere estudios de poblaciones diversas.
  • Este estudio investiga el NE, el estado de portador del gen APOE ε4 y la función cognitiva en adultos portorriqueños (PR) con pTau181 plasmático elevado, un marcador de PEA.

Objetivo del estudio:

  • Examinar la relación entre el nivel educativo (NE) y la función cognitiva en adultos mayores portorriqueños con patología de la enfermedad de Alzheimer (PEA) elevada.
  • Investigar cómo el genotipo APOE ε4 modifica la asociación entre el NE y la función cognitiva en presencia de PEA.

Principales métodos:

  • Se analizaron datos de 793 adultos mayores portorriqueños con pTau181 plasmático elevado.
  • La función cognitiva se evaluó mediante el compuesto funcional de la escala de calificación de demencia clínica (CDR-FUNC).
  • El NE se dicotomizó (alto >9 años, bajo ≤9 años); las asociaciones con CDR-FUNC y el estado de portador del gen APOE ε4 se probaron mediante pruebas U de Mann-Whitney.

Principales resultados:

  • Un menor NE se asoció significativamente con una peor función cognitiva en individuos con pTau181 elevado (p=6.5×10⁻⁴).
  • La asociación entre el NE y la función cognitiva fue más fuerte en los portadores del gen APOE ε4 (p=1.52×10⁻³) que en los no portadores (p=0.06).
  • En el grupo con bajo NE, los portadores del gen APOE ε4 mostraron una peor función cognitiva que los no portadores (p=0.045), una diferencia que no se observó en el grupo con alto NE.

Conclusiones:

  • El nivel educativo (NE) puede potenciar la reserva cognitiva y la resiliencia a la patología de la enfermedad de Alzheimer (PEA) en la población portorriqueña.
  • Un NE alto proporciona una ventaja cognitiva contra el declive funcional, incluso con pTau181 elevado.
  • Los portadores del gen APOE ε4 con bajo NE enfrentan un mayor riesgo de declive cognitivo en comparación con aquellos con NE alto, lo que enfatiza la interacción entre la genética y la educación.
