Sus antecedentes:

  • La evaluación actual de la arteriolosclerosis cerebral se basa en escalas subjetivas y semicuantitativas del examen histológico manual.
  • Los métodos manuales presentan una fiabilidad interevaluador limitada y carecen de métricas cuantitativas precisas.
  • El análisis específico de la región y de alta resolución a menudo no es práctico manualmente.

Objetivo del estudio:

  • Desarrollar y validar un algoritmo basado en aprendizaje automático (ML) para el análisis morfométrico automatizado de vasos sanguíneos arterioloscleróticos.
  • Introducir la segmentación de arteriolosclerosis (ArtS) y el extractor de índice y grosor de esclerosis (SITE) para la evaluación cuantitativa en imágenes de cortes completos (WSI).

Principales métodos:

  • WSI cerebrales digitalizadas teñidas con hematoxilina y eosina de tres bancos de cerebros.
  • Se entrenaron tres modelos de ML dentro de ArtS para la detección de vasos sanguíneos, la clasificación de arteriolosclerosis y la segmentación de la pared/lumen del vaso.
  • Se utilizó la herramienta SITE para extraer índices de esclerosis y grosores de pared de los vasos segmentados.

Principales resultados:

  • ArtS demostró un rendimiento sólido en la detección de vasos sanguíneos (AUC-ROC de hasta 0.79) y la clasificación de arteriolosclerosis (precisión de hasta 0.94).
  • La segmentación de vasos arterioloscleróticos logró puntuaciones Dice de hasta 0.73 y AUC-ROC de hasta 0.92.
  • SITE calculó con éxito métricas cuantitativas comparables a las evaluaciones de expertos.

Conclusiones:

  • El algoritmo ArtS y SITE muestra un potencial significativo para el análisis morfométrico cuantitativo de la arteriolosclerosis.
  • Estas herramientas de ML pueden mejorar los métodos actuales de evaluación humana de la patología vascular.
  • Una mayor optimización podría conducir a una valiosa herramienta de asistencia para neuropatólogos.
