  • Mayo Test Drive (MTD) es una herramienta de evaluación digital validada que comprende pruebas de memoria y velocidad de procesamiento.
  • Existe la necesidad de modelos predictivos de cambio cognitivo longitudinal en evaluaciones digitales.

Objetivo del estudio:

  • Desarrollar un modelo normativo condicional para predecir el cambio cognitivo longitudinal utilizando el MTD.
  • Validar este modelo predictivo en cohortes independientes de participantes cognitivamente no afectados (CU) y con deterioro cognitivo leve (MCI)/demencia (DEM).

Principales métodos:

  • Se desarrolló un modelo normativo condicional utilizando modelos lineales mixtos en 1355 participantes de CU con 2-4 evaluaciones de MTD.
  • Se incluyeron la edad, el sexo, la educación y la puntuación basal como predictores; se evaluaron variables adicionales para un aumento de r-cuadrado de >1%.
  • Se validó externamente el modelo en cohortes independientes de CU y MCI/DEM, comparando las puntuaciones observadas y las predichas.

Principales resultados:

  • La edad, la puntuación basal, el sexo y la educación fueron predictores significativos en el modelo final.
  • El modelo mostró una buena concordancia entre las puntuaciones observadas y las predichas en las cohortes de desarrollo y validación de CU.
  • El modelo demostró sensibilidad al deterioro cognitivo longitudinal, con puntuaciones residuales z significativamente peores en los participantes con MCI/DEM.

Conclusiones:

  • Se desarrollaron modelos normativos longitudinales para la evaluación MTD.
  • Estos modelos pueden identificar a personas que presentan un deterioro cognitivo longitudinal anormal.
  • Los hallazgos respaldan el uso de modelos basados en MTD para monitorizar trayectorias cognitivas.
