Sus antecedentes:

  • Las anomalías de neuroimagen relacionadas con amiloide (ARIA), específicamente ARIA-E (edema), son complicaciones reconocidas en pacientes que reciben tratamiento con lecanemab para la enfermedad de Alzheimer (EA) temprana.
  • La identificación y vigilancia temprana de ARIA son cruciales para garantizar la seguridad del paciente y optimizar la eficacia del tratamiento.
  • El acceso a la RM convencional de alto campo (AC) para la monitorización de ARIA a menudo está restringido por desafíos logísticos y altos costos.

Objetivo del estudio:

  • Evaluar la eficacia de la RM portátil de bajo campo (BC) para detectar y monitorizar ARIA-E en pacientes con EA temprana en tratamiento con lecanemab.
  • Comparar el rendimiento diagnóstico de la RM de BC con la RM estándar de alto campo (AC) para la evaluación de ARIA-E.

Principales métodos:

  • Veinte pacientes con EA temprana en tratamiento con lecanemab se sometieron a 40 exploraciones de RM de BC (0,064 T).
  • Las exploraciones de RM de AC (3 T) más cercanas en el tiempo sirvieron como estándar de referencia.
  • Cuatro neurorradiólogos evaluaron las exploraciones de RM de BC para detectar ARIA-E, y las dimensiones/volúmenes de las lesiones se segmentaron manualmente y se correlacionaron con los hallazgos de la RM de AC.

Principales resultados:

  • La RM de bajo campo identificó correctamente ARIA-E en 2 de 20 pacientes e identificó con precisión los casos ARIA-negativos en 18 de 20.
  • Se observaron correlaciones sólidas entre las mediciones de RM de bajo campo y de alto campo tanto para la dimensión máxima de ARIA-E (r=0,9239, p=0,0001) como para el volumen (r=0,9562, p<0,0001).

Conclusiones:

  • La RM portátil de BC muestra potencial para detectar y monitorizar ARIA-E en pacientes tratados con lecanemab, superando potencialmente los problemas de accesibilidad de la RM de AC.
  • A pesar de la menor calidad de imagen que la RM de AC, la capacidad de la RM de BC para facilitar decisiones terapéuticas rápidas puede tener un valor clínico significativo.
  • Se necesitan estudios más amplios para confirmar estos hallazgos, optimizar los protocolos de RM de BC para el cribado de ARIA y establecer su papel en la atención rutinaria del paciente.
