Sus antecedentes:

  • El deterioro conductual leve (MBI) y la fragilidad física son comunes en adultos mayores, a menudo precediendo a la demencia.
  • El vínculo entre MBI, fragilidad y patologías subyacentes como la beta-amiloide (Aß) y la enfermedad cerebrovascular está poco estudiado.
  • Esta investigación investiga las asociaciones de MBI y fragilidad en una cohorte del sudeste asiático con riesgo de demencia debido a patología de Aß y/o cerebrovascular.

Objetivo del estudio:

  • Examinar la asociación entre MBI y fragilidad física en individuos con riesgo sintomático de demencia.
  • Explorar estas asociaciones en relación con la patología subyacente de beta-amiloide (Aß) y/o cerebrovascular.
  • Identificar posibles marcadores conductuales para el riesgo de deterioro cognitivo.

Principales métodos:

  • Un estudio transversal de 293 adultos que viven en la comunidad con deterioro cognitivo subjetivo o deterioro cognitivo leve.
  • MBI evaluado mediante el Mild Behavioral Impairment Checklist (MBI-C); fragilidad física por el Fenotipo de Fragilidad de Fried.
  • Patología determinada por la relación Aß42/40 en plasma (Aß) y la calificación modificada de Fazekas (cerebrovascular); los participantes se categorizaron en grupos de patología pura de Aß o mixta.

Principales resultados:

  • En el grupo de patología mixta, los individuos prefrágiles mostraron puntuaciones significativamente más bajas en MBI-C en los dominios de estado de ánimo, impulsos y creencias en comparación con los individuos no frágiles.
  • No se encontraron diferencias significativas en las puntuaciones de MBI-C entre los individuos no frágiles y prefrágiles en el grupo de patología pura de Aß.
  • La puntuación total de MBI-C aumentada y dominios específicos (estado de ánimo, impulsos, creencias) se asociaron significativamente con una mayor probabilidad de ser prefrágil en el grupo de patología mixta, incluso después de ajustes.

Conclusiones:

  • Existe una asociación significativa entre MBI y fragilidad en individuos con patología vascular-amiloide mixta.
  • El empeoramiento de los síntomas de MBI se correlaciona con una mayor probabilidad de prefragilidad en esta población.
  • Estos hallazgos sugieren un componente conductual en la progresión del deterioro cognitivo, que requiere una mayor investigación para estrategias de detección e intervención tempranas.
