Objetivo del estudio:

  • Evaluar la asociación entre el uso de inhibidores del cotransportador sodio-glucosa-2 (SGLT2i) y el riesgo de demencia.; Evaluar esta asociación en individuos con insuficiencia cardíaca y diabetes comórbida.; Identificar estrategias preventivas potenciales para la demencia en grupos de pacientes de alto riesgo.

Principales métodos:

  • Estudio de cohorte retrospectivo utilizando bases de datos administrativas de salud en Ontario, Canadá.; Inclusión de individuos de ≥66 años con insuficiencia cardíaca y diabetes, que iniciaron SGLT2i o DPP4i.; Se utilizaron pesos de estratificación por puntuación de propensión y modelos de regresión de Cox para el análisis, ajustando por factores de confusión y riesgos competitivos.

Principales resultados:

  • La iniciación de SGLT2i se asoció con un riesgo 26% menor de demencia incidente en comparación con DPP4i (HR 0.74).; Se observó una reducción del riesgo de demencia con SGLT2i en subgrupos, incluidos aquellos con fibrilación auricular y de ≥75 años.; El análisis secundario según lo tratado indicó una mayor reducción del riesgo de demencia con el uso de SGLT2i.

Conclusiones:

  • La iniciación de SGLT2i demuestra una asociación significativa con un menor riesgo de demencia en pacientes con insuficiencia cardíaca y diabetes.; Los hallazgos sugieren un papel neuroprotector potencial para SGLT2i en poblaciones de alto riesgo.; Se requieren ensayos controlados aleatorizados adicionales para confirmar estos hallazgos prometedores.
