Sus antecedentes:

Objetivo del estudio:

  • Analizar las pruebas de función hepática (PFH), los parámetros hematológicos y los factores clínicos en pacientes con cirrosis.
  • Evaluar la utilidad diagnóstica de las PFH para identificar la cirrosis.
  • Comprender las limitaciones de las PFH en la enfermedad hepática compensada.

Principales métodos:

  • Estudio observacional retrospectivo de 251 pacientes con cirrosis confirmada.
  • Inclusión de PFH, hemograma completo, índice internacional normalizado (INR) y puntuación de albúmina-bilirrubina (ALBI).
  • Análisis de subgrupos comparando pacientes con PFH normales versus anormales.

Principales resultados:

  • El 37% de los pacientes con cirrosis tuvieron PFH normales; este porcentaje aumentó al 48% sin correlatos clínicos.
  • La puntuación de albúmina-bilirrubina (ALBI) fue normal en el 59,1% de todos los pacientes y en el 83,3% del subgrupo con PFH normales.
  • Se observó una relación elevada de AST:ALT en la cohorte.

Conclusiones:

  • Las PFH estándar por sí solas son insuficientes para diagnosticar una proporción significativa de pacientes con cirrosis.
  • La cirrosis compensada puede presentarse con PFH normales, lo que subraya las limitaciones de las pruebas.
  • Se necesita un enfoque multiparamétrico que integre indicadores clínicos y herramientas no invasivas para la detección temprana de la cirrosis.
