Sus antecedentes:

  • La gammagrafía cardíaca con yodo-131-meta-yodobencilguanidina (131I-MIBG) evalúa la función del sistema nervioso simpático cardíaco.; Se observa una reducción de la captación miocárdica de 131I-MIBG en algunas afecciones neurodegenerativas.

Objetivo del estudio:

  • Comparar la denervación simpática en el miocardio en diversas enfermedades neurodegenerativas.; Investigar las diferencias en la función autonómica cardíaca en la Enfermedad de Parkinson (EP), Demencia por Enfermedad de Parkinson (PDD), Demencia con Cuerpos de Lewy (DLB), Atrofia Multisistémica (MSA), Insuficiencia Autonómica Pura (PAF), Parálisis Supranuclear Progresiva (PSP) y Síndrome Parkinsoniano (PDS).

Principales métodos:

  • Se reclutaron 222 pacientes con EP, 37 con PDD, 10 con DLB, 116 con MSA, 14 con PAF, 9 con PSP y 63 con PDS.; Se calculó la relación corazón-mediastino (relación H/M) a los 15 minutos y 4 horas después de la inyección de la gammagrafía cardíaca con 131I-MIBG.; Se compararon las relaciones H/M entre los diferentes grupos de enfermedades neurodegenerativas.

Principales resultados:

  • Los pacientes con EP, PDD y DLB mostraron relaciones H/M significativamente más bajas en comparación con PAF, MSA, PSP y PDS (p < 0,05).; Los grupos de PDD y DLB exhibieron relaciones H/M significativamente reducidas en comparación con el grupo de EP (p < 0,05).; No se encontraron diferencias significativas en las relaciones H/M entre los grupos de PDD y DLB.

Conclusiones:

  • La gammagrafía cardíaca con 131I-MIBG es valiosa para comprender la participación del sistema nervioso autónomo en las enfermedades neurodegenerativas.; Los pacientes con PDD y DLB demuestran una menor captación cardíaca de MIBG que los pacientes con EP, lo que sugiere cambios patológicos distintos.; Los hallazgos ofrecen nuevas perspectivas sobre las posibles diferencias en la patología entre EP, PDD y DLB.
