Sus antecedentes:

  • La evaluación ecológica momentánea (EMA) es una herramienta valiosa para evaluar las preocupaciones cognitivas subjetivas (SCC) sin depender de la memoria de recuerdo.
  • Las SCC elevadas se asocian con un mayor riesgo de deterioro cognitivo, sin embargo, la investigación sobre las trayectorias conjuntas de las SCC y la función cognitiva es limitada.

Objetivo del estudio:

  • Investigar las asociaciones entre las trayectorias longitudinales de las preocupaciones cognitivas subjetivas (SCC) y la función cognitiva utilizando la EMA basada en teléfonos inteligentes.
  • Explorar cómo los diferentes patrones de SCC se relacionan con los cambios en las puntuaciones del Montreal Cognitive Assessment telefónico (T-MoCA) en adultos mayores.

Principales métodos:

  • Se utilizó la EMA basada en teléfonos inteligentes para el reporte diario de SCC durante 14 días y evaluaciones anuales de T-MoCA en 219 adultos mayores que viven en la comunidad.
  • Se emplearon modelos mixtos lineales de clases latentes para identificar las trayectorias de SCC, ajustando por factores demográficos y clínicos.
  • Se analizaron las trayectorias de T-MoCA en los grupos de SCC utilizando modelos mixtos lineales con splines de quiebre para tener en cuenta los efectos de aprendizaje.

Principales resultados:

  • Se identificaron dos grupos de trayectorias de SCC: uno con SCC consistentemente bajas y otro con SCC crecientes.
  • No se observaron diferencias basales en T-MoCA entre los grupos, pero el grupo de SCC crecientes mostró una disminución en las puntuaciones de T-MoCA después de tres años, a diferencia del grupo de SCC bajas.

Conclusiones:

  • Las trayectorias longitudinales de las preocupaciones cognitivas subjetivas se asocian con trayectorias distintas de rendimiento cognitivo.
  • El aumento de las SCC puede servir como un predictor de futuras disminuciones en la función cognitiva objetiva.
