Sus antecedentes:

  • Las evaluaciones cognitivas remotas ofrecen información única más allá de las pruebas tradicionales para la enfermedad de Alzheimer (EA).
  • Las preocupaciones sobre la familiaridad con la tecnología y la usabilidad en adultos mayores y en personas con enfermedad de Alzheimer asociada al síndrome de Down (DSAD) se están abordando mediante la implementación exitosa de herramientas digitales.
  • Las evaluaciones remotas aumentan la accesibilidad y reducen la carga del participante, lo que resulta beneficioso para diversas poblaciones.

Objetivo del estudio:

  • Destacar un resultado cognitivo novedoso en la EA esporádica factible solo a través de evaluaciones digitales remotas.
  • Describir los esfuerzos para traducir estos métodos de evaluación remota para estudios globales de DSAD.
  • Analizar la variabilidad cognitiva en escalas de tiempo diarias y momentáneas en poblaciones de EA y DSAD.

Principales métodos:

  • Se utilizaron datos del Knight Alzheimer's Disease Research Center (N=377) y del estudio Ambulatory Research in Cognition-Down Syndrome (ARC-DS) (N=21).
  • Los participantes del Knight ADRC completaron evaluaciones cognitivas remotas diarias durante siete días.
  • Los participantes del ARC-DS completaron una prueba remota modificada varias veces, tanto en el sitio como en casa, para evaluar la usabilidad y los patrones de variabilidad.

Principales resultados:

  • La variabilidad cognitiva difirió significativamente a lo largo de la semana de pruebas en los participantes del Knight ADRC.
  • Las personas con síntomas tempranos de EA (CDR 0.5) mostraron una mayor variabilidad cognitiva diaria y momentánea que las personas cognitivamente normales (CDR 0).
  • Los portadores de APOE e4 con riesgo genético de EA exhibieron una variabilidad diaria y momentánea significativamente mayor que los no portadores.
  • Los participantes en el estudio DSAD completaron con éxito las pruebas remotas de forma independiente, mostrando patrones de variabilidad similares.

Conclusiones:

  • Las evaluaciones cognitivas remotas capturan eficazmente cambios cognitivos únicos en la enfermedad de Alzheimer.
  • Estos métodos digitales son adecuados para evaluar tanto la EA esporádica como la EA asociada al síndrome de Down.
