Sus antecedentes:

  • El delirio es un trastorno cognitivo agudo en adultos mayores, relacionado con resultados adversos, incluido el futuro declive cognitivo.

Objetivo del estudio:

  • Determinar el impacto cognitivo a 1 año del delirio posoperatorio en pacientes sometidos a reparación de fractura de cadera (RRC).
  • Probar la hipótesis de que el delirio tiene un mayor impacto cognitivo en individuos sin deterioro cognitivo basal en comparación con aquellos con deterioro preexistente.

Principales métodos:

  • Análisis de evaluaciones cognitivas de 200 pacientes con RRC (edad ≥65, MMSE ≥15) del ensayo STRIDE.
  • Adjudicación del estado de delirio y la Escala de Deterioro Clínico (CDR) por un panel de consenso.
  • Evaluación longitudinal de las puntuaciones del Mini-Mental State Exam (MMSE) en el momento basal, a 1 mes y a 1 año utilizando un modelo lineal de spline de intercepto aleatorio.

Principales resultados:

  • El delirio ocurrió en el 36.5% de los pacientes con RRC.
  • Los pacientes que experimentaron delirio mostraron una mayor tasa de declive de la MMSE durante 1 año, especialmente aquellos sin deterioro cognitivo basal (CDR=0).
  • Una mayor gravedad del delirio se correlacionó con un declive más rápido de la MMSE en el subgrupo cognitivamente intacto.

Conclusiones:

  • El delirio posoperatorio y su gravedad afectan significativamente las trayectorias cognitivas en adultos mayores después de la RRC.
  • Los individuos cognitivamente intactos parecen ser los más vulnerables a las consecuencias cognitivas a largo plazo del delirio.
  • La prevención del delirio es crucial para mantener la función cognitiva en pacientes ancianos sometidos a cirugía de fractura de cadera.
