Sus antecedentes:

  • La enfermedad de Alzheimer de herencia autosómica dominante (EAAD) presenta desafíos únicos en el desarrollo de fármacos.; Las personas cognitivamente sanas con EAAD ofrecen un modelo para estudiar la enfermedad de Alzheimer (EA) esporádica temprana.; El consorcio CPAD tiene como objetivo acelerar el desarrollo de fármacos para la EA mediante la integración de datos clínicos y observacionales.

Objetivo del estudio:

  • Modelar la dinámica fármaco-enfermedad-ensayo para la EAAD.; Investigar las patologías de tau y amiloide en la EA temprana.; Comprender las relaciones biomarcador-cognición y la progresión de la enfermedad.

Principales métodos:

  • Utilizó datos del ensayo Colombia de Prevención del Alzheimer de la Iniciativa de Prevención del Alzheimer para la EAAD.; Empleó un ensayo clínico controlado con placebo de crenezumab en miembros del linaje PSEN1 E280A.; Aplicó modelos lineales mixtos para analizar resultados longitudinales y datos de PET de tau/amiloide regional.

Principales resultados:

  • Identificó patrones distintos de acumulación de tau regional en portadores de mutaciones de EAAD frente a no portadores.; Observó variaciones en la dinámica basal de amiloide y efectos demográficos en la acumulación de tau.; Diferenció las características de progresión cognitiva entre la EAAD y la EA esporádica, revelando mecanismos neurodegenerativos.

Conclusiones:

  • Caracterizó patrones únicos de acumulación de tau e interacciones amiloide-tau en portadores de mutaciones de EAAD.; Proporcionó información sobre la progresión temprana de la enfermedad y los mecanismos neurodegenerativos para la EAAD y la EA esporádica.; Validó un marco sólido para avanzar en la investigación terapéutica en la enfermedad de Alzheimer.
