Sus antecedentes:

  • Las enfermedades pulmonares intersticiales (EPI) abarcan más de 200 subtipos con resultados variables.
  • Existen datos limitados del mundo real sobre la prevalencia y las características de las EPI en Alemania.
  • La comprensión de la epidemiología de las EPI es crucial para mejorar la atención y los resultados de los pacientes.

Objetivo del estudio:

  • Describir las características epidemiológicas y clínicas de los subtipos de EPI en Alemania.
  • Analizar los patrones de diagnóstico y tratamiento dentro de una cohorte de pacientes del mundo real.
  • Identificar los factores que influyen en el tiempo hasta el diagnóstico de las enfermedades pulmonares intersticiales.

Principales métodos:

  • Inscripción prospectiva de 601 pacientes con diversos subtipos de EPI en 32 centros alemanes.
  • La recopilación de datos incluyó características basales, métodos de diagnóstico y estrategias de tratamiento.
  • Se emplearon análisis descriptivos y pruebas estadísticas (suma de rangos con signo de Wilcoxon).

Principales resultados:

  • La sarcoidosis (26,6%) y la fibrosis pulmonar idiopática (FPI) (25,3%) fueron las EPI más comunes.
  • Se observó hipertensión pulmonar en el 8,7% de los pacientes, con mayor frecuencia en subtipos específicos de EPI.
  • La discusión multidisciplinaria redujo significativamente el tiempo de diagnóstico (31,6 frente a 49,2 meses).

Conclusiones:

  • El registro EXCITING-ILD ofrece una visión general representativa de las EPI en Alemania.
  • Los hallazgos resaltan las frecuencias de los subtipos de EPI, las limitaciones de los pacientes y las comorbilidades.
  • El estudio subraya la importancia de los enfoques multidisciplinarios en el diagnóstico de las EPI.
