Sus antecedentes:

  • Los residentes de atención a largo plazo experimentan frecuentemente deterioro cognitivo y síntomas conductuales y psicológicos de la demencia (SCPD).
  • El manejo actual de los SCPD en la atención a largo plazo a menudo es subóptimo, lo que lleva a una dependencia excesiva de los medicamentos psicotrópicos y a un uso insuficiente de los enfoques no farmacológicos.
  • Anteriormente se desarrolló una Vía de Atención Integrada (ICP) que combina métodos farmacológicos y no farmacológicos para el manejo de los SCPD.

Objetivo del estudio:

  • Implementar y evaluar una Vía de Atención Integrada mejorada con tecnología (Tech-ICP) para el manejo de los SCPD en entornos de atención a largo plazo.
  • Evaluar la efectividad de Tech-ICP utilizando un diseño de ensayo pragmático.
  • Integrar la monitorización y las intervenciones basadas en tecnología en los modelos de atención de atención a largo plazo.

Principales métodos:

  • Tech-ICP integra herramientas de evaluación, sistemas de monitorización y tratamiento algorítmico basado en los principios de la ICP.
  • La intervención incluye paneles de control para el personal clínico, dispositivos portátiles de monitorización biométrica y realidad virtual (RV) para terapia de reminiscencia y formación del personal.
  • Se llevará a cabo un ensayo pragmático de tres años, en forma de escalonamiento por fases, en 40 centros de atención a largo plazo en Toronto, que incluirá fases de evaluación de la preparación, implementación y sostenibilidad.

Principales resultados:

  • Se espera que Tech-ICP mejore las capacidades de los equipos clínicos en el tratamiento de los SCPD a través de la tecnología.
  • Los resultados previstos incluyen una reducción del uso inadecuado de medicamentos psicotrópicos y una mejora de la calidad de vida de los residentes de atención a largo plazo.
  • También se prevé que la intervención disminuya la carga y el agotamiento de los cuidadores, así como que reduzca las visitas a urgencias y las admisiones hospitalarias.

Conclusiones:

  • Tech-ICP ofrece un modelo sostenible para el manejo de los SCPD en la atención a largo plazo mediante la integración de intervenciones tecnológicas avanzadas.
  • Se espera que este enfoque mejore la calidad de la atención, optimice el uso de medicamentos y mejore el bienestar de los residentes y cuidadores.
  • Los hallazgos informarán el diseño de futuros ensayos pragmáticos para los SCPD en poblaciones de atención a largo plazo.
