Incidencia y factores de riesgo perioperatorios del delirio posoperatorio tras una cirugía urológica mayor

Antecedentes: El delirio postoperatorio (POD) es una de las complicaciones quirúrgicas más comunes en pacientes ancianos. Este estudio investigó la incidencia y los factores de riesgo perioperatorios del delirio postoperatorio tras cirugía urológica.Métodos: Se incluyó un total de 162 pacientes hombres de 50 años o más sometidos a cirugía urológica mayor electiva bajo anestesia general entre mayo de 2024 y marzo de 2025 en este estudio observacional prospectivo. Se evaluó el delirio mediante la CAM-ICU dos veces al día durante cinco días posoperatorios. Se compararon los grupos con y sin delirio, y se analizaron los predictores perioperatorios de delirio. Se utilizó análisis de regresión multivariante para identificar factores de riesgo independientes para el delirio posoperatorio (POD).Resultados: En general, el 16% de los pacientes desarrollaron delirio durante el período de seguimiento. Los pacientes con delirio posoperatorio (DPO) eran significativamente mayores (edad media, 73,3 ± 5,2 años frente a 66,3 ± 7,2,p< 0,001), tenían más comorbilidades y vivían en áreas rurales. La fibrilación auricular y la EPOC fueron particularmente significativas. La incidencia de delirio posoperatorio (POD) fue mayor en pacientes con consumo leve/moderado de alcohol en comparación con aquellos que nunca bebían. El análisis de factores intraoperatorios reveló una diferencia significativa entre los grupos en cuanto a la presencia de hipotensión intraoperatoria y transfusión sanguínea. Los pacientes con delirio presentaron dolor posoperatorio más severo. Se determinaron valores óptimos de corte para la edad (≥67,5 años), número de comorbilidades (≥2), puntuación previa a la cirugía en el test MMSE (≤25,5) y puntuación posoperatoria en la escala NRS (≥4,85) mediante curvas ROC. El análisis multivariado identificó como factores de riesgo independientes en esta cohorte: edad ≥ 67,5 años, EPOC, consumo leve/moderado de alcohol, puntuación previa a la cirugía en el test MMSE ≤ 25,5, hipotensión intraoperatoria y puntuación posoperatoria en la escala NRS ≥ 4,85.Conclusiones: Dado que algunos de los factores de riesgo mencionados pueden modificarse, es necesario enfatizar que la prevención del DPO es posible y debe ser una de las prioridades terapéuticas en pacientes mayores.
