Objetivo del estudio:
- Investigar los perfiles de mutación somática asociados con la ocurrencia de convulsiones en pacientes con astrocitoma.
- Identificar posibles impulsores genómicos de la epilepsia asociada a tumores.
Principales métodos:
- Se realizó una secuenciación del exoma completo en tejido tumoral de 40 pacientes con astrocitoma (25 con convulsiones, 15 sin ellas).
- Se analizaron los perfiles de mutación y se correlacionaron con el estado de convulsión, estratificados por el estado de mutación de IDH.
Principales resultados:
- No se observaron diferencias clínicas significativas entre los grupos con y sin convulsiones.
- Las mutaciones de CELSR1 se enriquecieron significativamente en el grupo con convulsiones dentro del subgrupo de astrocitoma silvestre de IDH (31,6 % frente a 0 %, p=0,028).
- Otros genes candidatos, como GSTT4, perdieron la asociación en los análisis de subgrupos.
Conclusiones:
- Las mutaciones de CELSR1 representan un nuevo contribuyente potencial a la epilepsia asociada a tumores en astrocitomas silvestres de IDH.
- La subagrupación molecular es importante para comprender el riesgo de convulsiones en los tumores cerebrales.
- Se justifican estudios funcionales y de validación adicionales de CELSR1 en la epileptogénesis.
