Endoscopic therapies for colonic diverticular bleeding: a systematic review and meta-analysis

Endoscopic treatments for colonic diverticular bleeding (CDB) show high success rates. Through-the-scope (TTS) clips had worse outcomes, while endoscopic detachable snare ligation (EDSL) had more adverse events.

- El sangrado diverticular colónico (SDC) es una causa frecuente de hemorragia gastrointestinal inferior aguda.
Objetivo del estudio:
- Revisar y meta-analizar sistemáticamente la eficacia y seguridad de diferentes opciones de tratamiento endoscópico para el SDC.
Principales métodos:
- Se buscaron en las bases de datos PubMed, Embase y Cochrane Library.
Principales resultados:
- Se incluyeron 21 estudios observacionales con 3161 participantes.
- La tasa de hemostasia exitosa combinada fue del 98,90 % (IC del 95 % 97,66-99,75 %) sin diferencias significativas en subgrupos.
- Los clips TTS mostraron peores resultados en cuanto a resangrado y tiempo de procedimiento; la EDSL tuvo la tasa más alta de eventos adversos (2,15 %).
Conclusiones:
- El tratamiento endoscópico para el SDC ofrece altas tasas de hemostasia inmediata en todas las modalidades.
