    Sus antecedentes:

    • La oncología de precisión aprovecha los biomarcadores para tratamientos oncológicos personalizados.
    • La transcriptómica de ARN está infrautilizada para la predicción de la respuesta a fármacos debido a limitaciones de datos y escasez de modelos.

    Objetivo del estudio:

    • Desarrollar un marco robusto de aprendizaje automático para predecir la respuesta del paciente a diversas terapias contra el cáncer utilizando datos transcriptómicos previos al tratamiento.
    • Crear el conjunto de datos transcriptómicos más grande para la predicción de la respuesta a fármacos.

    Principales métodos:

    • Se ensambló un gran conjunto de datos (69 cohortes, 3729 pacientes) en nueve tipos de cáncer y seis terapias de primera línea.
    • Se desarrolló EXPRESSO (EXpression-Profile-RESponSe-Optimizer), un modelo de aprendizaje automático supervisado que integra dianas de fármacos y biomarcadores.
    • Se evaluó el rendimiento de EXPRESSO frente a 20 firmas transcriptómicas publicadas.

    Principales resultados:

    • EXPRESSO logró un rendimiento predictivo significativo (ROC-AUCs de 0.64–0.73, odds ratios de 2.4–4.6) en múltiples terapias.
    • El modelo superó a 20 firmas transcriptómicas existentes.
    • El análisis de robustez indicó mesetas de rendimiento para algunas terapias, lo que sugiere límites potenciales de los enfoques actuales de aprendizaje supervisado.

    Conclusiones:

    • Los datos transcriptómicos, cuando se analizan con aprendizaje automático avanzado como EXPRESSO, pueden predecir eficazmente la respuesta al tratamiento del cáncer.
    • Más datos y modelado mecanicista pueden mejorar el poder predictivo de los biomarcadores transcriptómicos para la oncología personalizada.
    • EXPRESSO representa un avance significativo en la utilización de datos de ARN para la selección de terapias contra el cáncer de precisión.
