Este estudio presenta un nuevo marco de presencia de ruido para electrocardiogramas (ECG) que adapta el filtrado al ruido detectado, mejorando la calidad de la señal y preservando mediciones cardíacas cruciales. Este filtrado adaptativo al ruido mejora la precisión diagnóstica para el análisis de ECG.

Sus antecedentes:

  • Los electrocardiogramas (ECG) son vitales para la monitorización cardíaca, pero son susceptibles al ruido, lo que degrada la calidad de la señal.
  • Los métodos convencionales de filtrado de ECG aplican una reducción de ruido uniforme, lo que puede distorsionar los segmentos de señal limpios y las características clínicas importantes.

Objetivo del estudio:

  • Desarrollar y evaluar un novedoso marco de presencia de ruido para el procesamiento de señales de ECG.
  • Adaptar las estrategias de filtrado en función de la presencia y el tipo de ruido, con el objetivo de minimizar la distorsión de la señal y preservar los parámetros clínicamente relevantes del ECG.

Principales métodos:

  • Se desarrolló un marco de presencia de ruido para detectar el ruido, clasificar su tipo y aplicar un filtrado a medida.
  • Se utilizó la estimación de densidad de kernel (KDE) para evaluar la influencia de la frecuencia de muestreo en la detección de ruido, identificando 500 Hz como óptima.
  • Se implementó un modelo Adaboost jerárquico y se comparó con clasificadores SVM, RF y ExtraTree para la precisión de la detección y clasificación del ruido.

Principales resultados:

  • El modelo Adaboost jerárquico logró una alta precisión ( [Fórmula: ver texto] ) en la detección de ruido y ( [Fórmula: ver texto] ) en la clasificación de ruido en siete conjuntos de datos.
  • El filtrado de perfil de ruido demostró un rendimiento superior, lo que resultó en la menor diferencia media del intervalo QT (2,50 ms) en comparación con el filtrado con presencia de ruido ( [Fórmula: ver texto] ms) y el filtrado agnóstico al ruido ( [Fórmula: ver texto] ms).
  • Se observaron mejoras significativas en las mediciones del intervalo QRS, reduciendo las diferencias de [Fórmula: ver texto] ms (agnóstico al ruido) a [Fórmula: ver texto] ms (con presencia de ruido) y 4,28 ms (perfil de ruido).

Conclusiones:

  • Adaptar el filtrado de ECG en función de la presencia y el tipo de ruido mejora significativamente la preservación de los parámetros clínicos, lo que conduce a mediciones de intervalo más fiables.
  • El marco desarrollado es adecuado para sistemas de ECG portátiles y puede extenderse a otras señales fisiológicas con un reentrenamiento adecuado.
  • Aunque se entrenó con ruido sintético, la aplicabilidad práctica del marco se ve respaldada por su eficacia en la preservación de las características diagnósticas del ECG.
