Resumen

Las pruebas de CD4 durante la hospitalización de personas con el virus de la inmunodeficiencia humana (VIH) a menudo no son útiles debido a la supresión temporal inducida por el estrés. Centrarse en la supresión viral y la adherencia a la TAR para una evaluación precisa del estado inmunológico.

Sus antecedentes:

Las pruebas de CD4 son estándar para las personas hospitalizadas con VIH (PWH).
Los estresores agudos y los medicamentos pueden disminuir temporalmente los recuentos de CD4, imitando la progresión de la enfermedad.
La mayoría de los PWH en terapia antirretroviral (TAR) tienen cargas virales suprimidas y sistemas inmunológicos reconstituidos.

Conclusiones:

Las pruebas rutinarias de CD4 en PWH hospitalizados a menudo no son útiles y pueden ser engañosas.
La interpretación requiere una cuidadosa consideración del contexto clínico, el porcentaje de CD4 y el estado de la TAR.
Se recomienda la consulta con especialistas en VIH cuando las pruebas de CD4 puedan afectar el manejo agudo.
