Blinding is a commonly used method of not telling participants which treatment a subject is receiving. Blinding is a critical part of a randomized control trial or RCT. It reduces the bias that affects the results. In an RCT, blinding is used in the form of a placebo. A placebo effect occurs when untreated subjects falsely believe they have received the treatment and report improved symptoms. A placebo or a dummy treatment is administered to subjects to negate the bias caused by such an effect.

Sus antecedentes:

  • Los escribas de inteligencia artificial (IA) ambiental ofrecen una solución potencial a la importante carga de documentación y al agotamiento del médico.
  • A pesar de su promesa, el impacto en el mundo real de los escribas de IA no ha sido evaluado rigurosamente en ensayos clínicos aleatorizados.

Objetivo del estudio:

  • Evaluar el impacto de dos aplicaciones de escriba de IA ambiental, Microsoft Dragon Ambient eXperience (DAX) Copilot y Nabla, en el tiempo de documentación del médico y el agotamiento.
  • Comparar la efectividad de DAX Copilot y Nabla frente a un grupo de control de atención habitual en un ensayo clínico aleatorizado pragmático.

Principales métodos:

  • Un ensayo clínico aleatorizado pragmático paralelo de tres grupos que involucró a 238 médicos de 14 especialidades.
  • Los médicos fueron asignados aleatoriamente 1:1:1 a DAX Copilot, Nabla o un grupo de control de atención habitual.
  • El resultado primario fue el cambio en el tiempo de escritura en la nota; los resultados secundarios incluyeron la carga de trabajo del médico y las puntuaciones de agotamiento.

Principales resultados:

  • El uso de Nabla se asoció con una disminución estadísticamente significativa del 9,5% en el tiempo dedicado a la nota en comparación con el grupo de control (P=0,02).
  • DAX Copilot no mostró un cambio significativo en el tiempo dedicado a la nota en comparación con el grupo de control (P=0,66).
  • Tanto los usuarios de DAX Copilot como los de Nabla informaron posibles mejoras en la carga de trabajo del médico, el agotamiento y las puntuaciones de agotamiento laboral, aunque los hallazgos requieren una mayor validación.

Conclusiones:

  • Nabla demostró una reducción significativa en el tiempo dedicado a la nota por parte del médico, lo que sugiere su eficacia para agilizar la documentación.
  • Ambos escribas de IA mostraron beneficios potenciales para el bienestar del médico, pero se necesitan ensayos multicéntricos adicionales para confirmar estos resultados secundarios.
  • Los médicos percibieron un rendimiento similar entre las dos plataformas de escribas de IA, lo que resalta la necesidad de una vigilancia continua con respecto a las imprecisiones ocasionales.
