Sus antecedentes:

  • El estreñimiento es una queja frecuente entre las mujeres que experimentan prolapso de órganos pélvicos.
  • Comprender el impacto de las intervenciones quirúrgicas en la función intestinal es crucial para la atención al paciente.

Objetivo del estudio:

  • Comparar las tasas de estreñimiento posoperatorio después de la colpopexia sacra abdominal frente a la cirugía de prolapso vaginal.
  • Evaluar el efecto de la reparación del prolapso en las puntuaciones de la Evaluación de los Síntomas de Estreñimiento del Paciente (PAC-SYM).

Principales métodos:

  • Estudio de cohorte prospectivo que involucra a pacientes sometidas a cirugía para prolapso apical.
  • Se utilizó el cuestionario de Evaluación de los Síntomas de Estreñimiento del Paciente (PAC-SYM) preoperatoriamente y a las 6 semanas y 6 meses después de la operación.
  • Se compararon los cambios en las puntuaciones de PAC-SYM entre los grupos quirúrgicos abdominal (colpopexia sacra) y vaginal utilizando análisis de regresión.

Principales resultados:

  • El estreñimiento preoperatorio estuvo presente en el 43% de los participantes.
  • No se observaron cambios significativos en las puntuaciones de PAC-SYM a las 6 semanas o 6 meses después de la operación en ninguno de los grupos quirúrgicos.
  • Una mayor proporción de pacientes en el grupo abdominal reportó cambios en los movimientos intestinales semanales a las 6 semanas en comparación con el grupo vaginal.

Conclusiones:

  • La cirugía de prolapso de órganos pélvicos no provoca un empeoramiento significativo de los síntomas de estreñimiento.
  • La colpopexia sacra parece tener efectos comparables sobre el estreñimiento que la cirugía de prolapso vaginal.
  • El estreñimiento sigue siendo un problema común para las mujeres con prolapso de órganos pélvicos, independientemente del abordaje quirúrgico.
