Resumen

La osteotomía metatarsiana flotante mínimamente invasiva trata eficazmente las úlceras del pie diabético (UFD) con una tasa de curación del 98 %. Este abordaje quirúrgico ofrece un cierre rápido y baja recurrencia, lo que la convierte en una opción prometedora para el manejo de las UFD.

Sus antecedentes:

  • Las úlceras del pie diabético (UFD) representan un desafío significativo debido a deformidades biomecánicas subyacentes.; Las UFD recurrentes o persistentes a menudo requieren intervenciones quirúrgicas avanzadas.

Objetivo del estudio:

  • Revisar sistemáticamente y meta-analizar la eficacia de la osteotomía metatarsiana flotante mínimamente invasiva para las UFD neuropáticas.; Evaluar las tasas de curación, el tiempo hasta el cierre y las tasas de complicaciones asociadas con el procedimiento.

Principales métodos:

  • Revisión sistemática y metaanálisis siguiendo las directrices PRISMA.; Se buscaron cuatro bases de datos de estudios sobre pacientes adultos con UFD neuropáticas tratadas con osteotomía metatarsiana flotante mínimamente invasiva.; Se incluyeron estudios con un seguimiento mínimo de 12 meses.

Principales resultados:

  • El análisis combinado de seis estudios (184 sujetos) mostró una tasa de curación de UFD del 98 %.; El tiempo medio hasta el cierre fue de 31,7 días; la recurrencia de la úlcera fue del 4 %.; Las tasas de complicaciones incluyeron lesiones por transferencia (14 %), falta de unión (14 %) e infección (7 %).

Conclusiones:

  • La osteotomía metatarsiana flotante mínimamente invasiva es una opción quirúrgica segura y eficaz para descargar las UFD neuropáticas.; El procedimiento demuestra altas tasas de curación y cierre rápido en comparación con los tratamientos conservadores.; Se necesitan ensayos controlados aleatorizados adicionales para validar los hallazgos y estandarizar las indicaciones.
