Sus antecedentes:

  • El envejecimiento se asocia con inflamación crónica de bajo grado (inflammaging), relacionada con el declive funcional.
  • La interleucina-6 (IL-6) es una citoquina proinflamatoria clave implicada en enfermedades relacionadas con la edad y el deterioro funcional.
  • La inhibición de la IL-6 puede ofrecer una estrategia terapéutica para mitigar el declive funcional relacionado con la edad.

Objetivo del estudio:

  • Evaluar la eficacia del clazkizumab, un inhibidor de la IL-6, en la mejora o el frenado del declive de la función física, cognitiva y vascular en adultos mayores.
  • Probar la hipótesis de la gerosciencia de que la intervención en el proceso de envejecimiento puede conducir a mejoras amplias de la salud.
  • Evaluar la seguridad y tolerabilidad del clazkizumab en esta población.

Principales métodos:

  • Un ensayo multicéntrico, aleatorizado, doble ciego, controlado con placebo (estudio RIGHT).
  • Inclusión de participantes de 70 años o más con función física baja a moderada y niveles elevados de IL-6.
  • Tratamiento con clazkizumab (inyección subcutánea de 5 mg cada 4 semanas durante 24 semanas) o placebo, con la velocidad de caminata como resultado primario.

Principales resultados:

  • Resultado primario: cambio en la velocidad de caminata de 400 m durante 24 semanas.
  • Resultados secundarios: evaluaciones de la función física (SPPB, consumo de oxígeno, fatigabilidad), función cognitiva, salud vascular, marcadores inflamatorios (IL-6, CRP) y seguridad.
  • Exploración de los cambios de 6 meses en la movilidad y otros aspectos de la salud.

Conclusiones:

  • El estudio RIGHT proporcionará datos cruciales sobre el potencial de la inhibición de la IL-6 para combatir el declive funcional relacionado con la edad.
  • Los hallazgos informarán sobre la validez de dirigirse a los mecanismos de envejecimiento para la extensión de la esperanza de vida saludable.
  • El estudio establecerá el perfil de seguridad y la eficacia del clazkizumab en una cohorte de adultos mayores.
