El stent que emite sirolimus (SES) de Abluminus DES+ no fue inferior al stent que emite everolimus (EES) de XIENCE en pacientes diabéticos sometidos a una intervención coronaria percutánea (ICP). Se observaron tasas más altas de revascularización y fallo de la lesión objetivo con Abluminus DES+ a los 12 meses.

Sus antecedentes:

  • La diabetes aumenta significativamente los riesgos de restenosis y eventos cardiovasculares adversos después de la intervención coronaria percutánea (ICP).
  • El stent de emisión de sirolimus (SES) de Abluminus DES+ tiene un recubrimiento abluminal y de superficie de globo para mejorar la administración del fármaco.
  • Es crucial comparar Abluminus DES+ SES con el stent de polímero duradero everolimus (EES) de XIENCE en pacientes diabéticos sometidos a una ICP.

Objetivo del estudio:

  • Comparar la eficacia y la seguridad del Abluminus DES+ SES frente al XIENCE EES en pacientes con diabetes sometidos a ICP.
  • Evaluar la no inferioridad de Abluminus DES+ SES para los parámetros clínicos clave a los 12 meses.

Principales métodos:

  • Un ensayo multicéntrico, prospectivo, abierto, aleatorizado y controlado (ABILITY Diabetes Global) en el que participaron 3032 pacientes con diabetes tipo 1 o tipo 2.
  • Los pacientes fueron asignados al azar 1:1 para recibir Abluminus DES+ SES o XIENCE EES.
  • Los parámetros de valoración primarios a los 12 meses fueron la revascularización de la lesión diana inducida por isquemia y la insuficiencia de la lesión diana, evaluados en la población por protocolo.

Principales resultados:

  • Abluminus DES+ SES no cumplía con los criterios de no inferioridad para la revascularización de la lesión diana inducida por isquemia (4,8% frente a 2,1%) y la insuficiencia de la lesión diana (9,7% frente a 6,2%) en comparación con XIENCE EES a los 12 meses.
  • El infarto de miocardio vascular objetivo ocurrió con mayor frecuencia en el grupo de Abluminus DES+ SES (5,2% frente a 3,1%).
  • No se observaron diferencias significativas en las muertes cardiovasculares o por cualquier causa entre los grupos.

Conclusiones:

  • Abluminus DES+ SES demostró tasas más altas de revascularización de la lesión diana y de insuficiencia de la lesión diana en comparación con XIENCE EES en pacientes diabéticos a los 12 meses de seguimiento.
  • Las tasas de eventos entre los 12 y los 24 meses fueron similares, lo que sugiere que no hubo divergencia tardía.
  • La optimización de los resultados en pacientes diabéticos sigue siendo un desafío, lo que requiere una innovación continua en la tecnología de stents y la farmacoterapia.
