Sus antecedentes:

  • La distrofia muscular de Duchenne (DMD) es un trastorno grave y progresivo del músculo esquelético.; Las complicaciones urológicas en la DMD incluyen hiperactividad del detrusor, hipocontractilidad de la vejiga, incontinencia urinaria, disinergia detrusor-esfínter y urolitiasis.; Esta revisión se centra en el manejo de la disfunción de la vejiga en pacientes con DMD.

Objetivo del estudio:

  • Investigar la incidencia de síntomas del tracto urinario inferior (STUI), problemas de defecación, infecciones del tracto urinario (ITU) y urolitiasis en niños con DMD.; Evaluar el impacto de los problemas urológicos en la calidad de vida de los niños con DMD.; Proporcionar recomendaciones para el manejo de la disfunción de la vejiga en la DMD.

Principales métodos:

  • Se realizó una búsqueda exhaustiva de la literatura en PubMed, Scopus, Embase, Cochrane y Web of Science.; Se seleccionaron nueve estudios no aleatorizados siguiendo las directrices PRISMA y se analizaron utilizando el marco PICO.; La calidad de los estudios se validó utilizando los criterios MINORS.

Principales resultados:

  • La prevalencia de STUI en pacientes con DMD varió del 32% al 85%, siendo la vacilación, la urgencia, la incontinencia y la frecuencia los más comunes.; Se informaron problemas de defecación concomitantes en el 32% al 70% de los pacientes.; La información sobre ITU, urolitiasis, calidad de vida y resultados del tratamiento fue escasa.

Conclusiones:

  • La DMD se asocia con una alta prevalencia de complicaciones urológicas, particularmente STUI y problemas de defecación.; Existe una falta significativa de datos sobre ITU, urolitiasis, calidad de vida y eficacia del tratamiento en la DMD.; Se recomienda un manejo multidisciplinario, incluyendo urólogos pediátricos, para abordar los STUI y la disfunción intestinal en la DMD.
