Sus antecedentes:

  • La enfermedad ocular tiroidea (EOT) es una condición autoinmune que afecta las órbitas.
  • El teprotumumab es un agente biológico novedoso utilizado para el tratamiento de la EOT activa.
  • Las irregularidades del ciclo menstrual son un efecto secundario potencial y poco reportado de las terapias sistémicas.

Objetivo del estudio:

  • Determinar la incidencia y características de las alteraciones del ciclo menstrual en pacientes que reciben teprotumumab para EOT.
  • Documentar los tipos de cambios menstruales y su relación temporal con las infusiones de teprotumumab.

Principales métodos:

  • Revisión retrospectiva de historias clínicas de 51 pacientes femeninas tratadas con teprotumumab en tres instituciones (2020-2023).
  • Se recopilaron datos demográficos de los pacientes, estado tiroideo, uso de anticonceptivos hormonales y datos del ciclo menstrual (tipo, inicio, duración).
  • También se registraron las puntuaciones de actividad clínica y los eventos adversos.

Principales resultados:

  • El 55% de los pacientes (28/51) experimentaron cambios menstruales.
  • Los cambios más frecuentes fueron amenorrea (75%) y oligomenorrea (21%).
  • Las irregularidades menstruales ocurrieron aproximadamente 58 días después de la primera infusión, con un retorno promedio a la menstruación normal de 155 días post-tratamiento.
  • El 14% no reanudó ciclos normales al momento del último seguimiento.

Conclusiones:

  • Las alteraciones del ciclo menstrual son un evento adverso común asociado con la terapia de teprotumumab en mujeres con EOT.
  • La amenorrea y la oligomenorrea son los cambios predominantes observados.
  • Los proveedores de atención médica deben informar a las pacientes sobre la posibilidad de irregularidades menstruales durante el tratamiento con teprotumumab.
