Sus antecedentes:

  • El éxito de la reconstrucción facial depende de la viabilidad del colgajo, lo que requiere un conocimiento detallado de la anatomía vascular.
  • La distribución regional y la importancia clínica de los perforantes de la piel facial no se comprenden completamente.

Objetivo del estudio:

  • Mapear la distribución y el diámetro de los perforantes faciales mediante disección cadavérica.
  • Ilustrar la utilidad clínica de estos perforantes en la cirugía reconstructiva.

Principales métodos:

  • Disección de 10 especímenes de cadáveres de hemicara después de la inyección de la arteria carótida con látex coloreado.
  • Examen microscópico y medición de los perforantes de la piel facial (≥0.10 mm) en 10 regiones faciales definidas.
  • Inclusión de dos casos clínicos para correlacionar los hallazgos anatómicos con los resultados reconstructivos.

Principales resultados:

  • Se observó la mayor densidad de perforantes en las regiones preauricular y bucal.
  • Variación regional significativa en el diámetro de los perforantes, siendo el más grande en la región bucal y el más pequeño en la región del párpado.
  • Identificación de perforantes dominantes en las áreas submentoniana y cigomático-orbitaria, con una densa red de microperforantes en otras áreas.

Conclusiones:

  • La perfusión de la piel facial implica perforantes axiales consistentes y una red difusa de microperforantes.
  • La comprensión de esta arquitectura vascular mejora las indicaciones para los colgajos basados en perforantes.
  • Proporciona una guía anatómica para una reconstrucción facial más segura, flexible y que preserve los tejidos.
