Sus antecedentes:
- La localización precisa para las compresiones torácicas (CT) en neonatos es fundamental para una reanimación eficaz.
- La ubicación precisa del ventrículo izquierdo (VI) en recién nacidos durante las CT no está bien definida.
Objetivo del estudio:
- Determinar la ubicación anatómica del ventrículo izquierdo (VI) en recién nacidos.
- Identificar las estructuras comprimidas por las compresiones torácicas (CT) en la región esternal inferior mediante ecocardiografía transtorácica.
Principales métodos:
- Estudio de cohorte observacional prospectivo en 50 recién nacidos.
- Se utilizó ecocardiografía transtorácica para visualizar las estructuras cardíacas.
- Se obtuvieron vistas ecocardiográficas estándar de acuerdo con las pautas de la American Society of Echocardiography.
Principales resultados:
- El ventrículo izquierdo (VI) rara vez se encontraba en el tercer borde esternal izquierdo (2%).
- En la mayoría de los recién nacidos, el VI se encontraba más lateralmente, en el cuarto (44%), quinto (50%) o sexto (4%) borde esternal izquierdo.
- Las compresiones torácicas en el tercio inferior del esternón pueden no dirigirse eficazmente al VI.
Conclusiones:
- Las pautas actuales de compresión torácica (CT) para recién nacidos pueden dirigirse inadvertidamente al corazón derecho, las venas cavas y la aorta en lugar del ventrículo izquierdo (VI).
- Se necesita más investigación para optimizar la colocación de las CT en la reanimación neonatal.
