Sus antecedentes:

Objetivo del estudio:

  • Desarrollar un marco analítico automatizado que integre características híbridas de radiómica y aprendizaje profundo a partir de imágenes de TC sin contraste.
  • Lograr la clasificación multiclase de patologías de la vejiga, incluyendo normalidad, cálculos, cáncer y cistitis.

Principales métodos:

  • Se realizó un análisis retrospectivo de 902 TC.
  • Se implementó un pipeline integrado que incluyó segmentación automática de la vejiga (3D-UNet), extracción de características híbridas (radiómica + aprendizaje profundo), selección de características (LASSO) y clasificación (XGBoost).
  • El rendimiento se evaluó utilizando AUROC con una estrategia de un vs. el resto y validación cruzada.

Principales resultados:

  • El marco logró altos AUROC de un vs. el resto: 0.94 para cálculos, 0.92 para cáncer, 0.90 para vejiga normal y 0.83 para cistitis.
  • El AUROC micro-promedio para la discriminación de cuatro clases fue 0.94.
  • Las características radiómicas fueron clave para la diferenciación de cálculos/normal, mientras que las características profundas fueron críticas para la clasificación de cáncer/cistitis.

Conclusiones:

  • El marco de análisis híbrido de TC demuestra un rendimiento clínicamente relevante en la clasificación automatizada multiclase de patologías de la vejiga.
  • Los roles complementarios de las características radiómicas y profundas ofrecen una ayuda diagnóstica interpretable.
  • Este marco muestra potencial para la integración en flujos de trabajo clínicos para apoyar el diagnóstico diferencial.
