Resumen

La intervención QUALI-DEC optimizó el uso de la cesárea en entornos de bajos recursos a través de una estrategia multifacética. Este enfoque empoderó a las mujeres y a los proveedores, mejorando la toma de decisiones y promoviendo prácticas basadas en la evidencia para mejores resultados de atención materna.

Sus antecedentes:

Las tasas de cesárea son una preocupación mundial, particularmente en países de ingresos bajos y medianos (LMIC).
La optimización del uso de la cesárea requiere estrategias multifacéticas que aborden factores del proveedor, del paciente y organizacionales.
La lista de verificación Plantilla para la Descripción y Replicación de Intervenciones (TIDieR) ayuda a la reproducibilidad de la intervención.

Objetivo del estudio:

Describir la intervención Uso Apropiado de la Cesárea a Través de la Calidad en la Toma de Decisiones (QUALI-DEC).
Detallar los componentes de la intervención utilizando la lista de verificación TIDieR para su replicación y escalamiento en países de ingresos bajos y medianos (LMIC).
Optimizar el uso de la cesárea a través de procesos mejorados de toma de decisiones.

Principales métodos:

La intervención QUALI-DEC se implementó en 32 hospitales de Argentina, Burkina Faso, Tailandia y Viet Nam (julio de 2022 - abril de 2024).
Los componentes clave incluyeron líderes de opinión, auditoría y retroalimentación (Sistema de Clasificación de los Diez Grupos de Robson), una Herramienta de Análisis de Decisiones y compañía durante el parto.
Los procesos de implementación, la capacitación, los recursos y las adaptaciones se documentaron sistemáticamente siguiendo la lista de verificación TIDieR.

Principales resultados:

Los líderes de opinión fueron cruciales para la capacitación, las auditorías y la implementación sostenida.
Los trabajadores de la salud recibieron capacitación sobre algoritmos clínicos, sistemas de auditoría, Herramienta de Análisis de Decisiones y el modelo de compañía de la OMS.
La intervención, apoyada por tecnología en línea, demostró ser factible en diversos contextos con adaptaciones culturales y de infraestructura.

Conclusiones:

El uso apropiado de la cesárea es complejo, influenciado por las mujeres, los proveedores y los factores organizacionales.
La intervención QUALI-DEC ofrece una estrategia pragmática y basada en equipos para la toma de decisiones centrada en el paciente y basada en la evidencia.
La descripción detallada a través de la lista de verificación TIDieR facilita la replicación y el monitoreo en unidades de maternidad de países de ingresos bajos y medianos.
