La recuperación del accidente cerebrovascular varía significativamente según la gravedad inicial y los factores individuales del paciente. Comprender estas trayectorias dinámicas de recuperación mejora el pronóstico y la atención personalizada al paciente.

Sus antecedentes:
- El pronóstico convencional del accidente cerebrovascular se basa en puntos de tiempo fijos, a menudo a los 3 meses posteriores al accidente cerebrovascular. Este enfoque puede no capturar completamente la naturaleza dinámica y heterogénea de la recuperación del accidente cerebrovascular. La identificación de los factores que influyen en los patrones de recuperación diferencial es crucial para la medicina personalizada.
Objetivo del estudio:
- Modelar las trayectorias de recuperación funcional en múltiples puntos de tiempo posteriores al accidente cerebrovascular. Analizar los patrones de recuperación basados en la gravedad inicial del accidente cerebrovascular. Identificar los factores clínicos asociados con trayectorias de recuperación distintas.
Principales métodos:
- Análisis de datos de un registro prospectivo de accidentes cerebrovasculares de 11 años. Inclusión de pacientes con accidente cerebrovascular isquémico con puntuaciones de la escala de Rankin modificada a los 1, 3, 6 y 12 meses. Estratificación por gravedad del accidente cerebrovascular utilizando la Escala de Accidente Cerebrovascular NIH (NIHSS) y modelado con ecuaciones de estimación generalizadas.
Principales resultados:
- Se analizaron 6.965 pacientes, estratificados en grupos de accidente cerebrovascular leve (NIHSS <5), moderado (5-15) y grave (NIHSS >15). Se observaron trayectorias de recuperación distintas, con la mayor parte de la mejora en los primeros 3 meses, seguida de un progreso más lento. La edad, el sexo, la terapia de reperfusión, la diabetes, la ESRD, la anemia, la leucocitosis, el ACV previo y la WMH impactaron significativamente los patrones de recuperación.
Conclusiones:
- La recuperación del accidente cerebrovascular es un proceso dinámico y heterogéneo. Los perfiles de referencia de los pacientes dictan trayectorias de recuperación funcional distintas. Un enfoque basado en trayectorias mejora la precisión pronóstica e informa el asesoramiento personalizado al paciente y la investigación sobre los mecanismos de recuperación.
