  • La implementación de programas estandarizados para procedimientos el mismo día tiene como objetivo mejorar la eficiencia de la atención al paciente.
  • Comprender los factores que influyen en las ofertas de procedimientos el mismo día es crucial para optimizar la prestación de atención médica.

Objetivo del estudio:

  • Identificar los factores del paciente y de diagnóstico asociados con la oferta de procedimientos el mismo día.
  • Evaluar la efectividad de un programa estandarizado de procedimientos el mismo día.

Principales métodos:

  • Estudio de mejora de la calidad que analiza datos de 315 pacientes sometidos a procedimientos mamarios.
  • Se utilizó el modelado logístico nominal para evaluar las asociaciones entre las características del paciente/diagnóstico y la logística de los procedimientos el mismo día.
  • Se registraron datos demográficos de los pacientes, detalles del examen y ocurrencias de procedimientos 'adicionales'.

Principales resultados:

  • Se ofreció una biopsia el mismo día al 53% de los pacientes; se realizaron el 11% de procedimientos 'adicionales'.
  • Los horarios de recomendación matutinos y las biopsias guiadas por ecografía se vincularon con tasas más altas de ofertas el mismo día.
  • Las aspiraciones guiadas por ecografía, las colecciones de líquidos y los hallazgos BI-RADS 5 aumentaron la probabilidad de procedimientos 'adicionales'.

Conclusiones:

  • Se recomienda la programación temprana de los estudios de diagnóstico, especialmente por la mañana, para las instituciones que ofrecen servicios el mismo día.
  • Priorizar a los pacientes con lesiones, abscesos, seromas o factores psicosociales altamente sospechosos puede mejorar aún más el acceso a la atención el mismo día.
  • La optimización de la logística de programación puede reducir los retrasos y mejorar el manejo oportuno de los pacientes.
