- La gangrena de Fournier (FG) es una fasciitis necrosante grave que afecta las áreas perineal y genital.; Las presentaciones atípicas de FG pueden oscurecer su causa subyacente.
Objetivo del estudio:
- Informar un caso raro de gangrena de Fournier secundaria a diverticulitis sigmoidea.; Enfatizar la importancia de considerar fuentes intraabdominales en casos inusuales de FG.
Principales métodos:
- Informe de caso de un paciente varón de unos 60 años con FG.; El estudio diagnóstico incluyó imágenes transversales.; El tratamiento implicó desbridamiento escrotal de emergencia, sigmoidectomía, antibióticos y cierre asistido por vacío.
Principales resultados:
- El paciente se presentó con shock séptico, necrosis escrotal y secreción fecal.; Las imágenes confirmaron la perforación retroperitoneal del colon sigmoide que se extendía hasta el escroto.; Se logró una recuperación exitosa mediante intervención quirúrgica rápida y cuidados de apoyo.
Conclusiones:
- Se debe considerar la patología intraabdominal, como la diverticulitis sigmoidea, en presentaciones atípicas de FG.; El diagnóstico oportuno mediante imágenes y el manejo multidisciplinario son cruciales para resultados favorables en FG.
