Resumen
La estereoencefalografía talámica (sEEG) es segura y precisa en pacientes con epilepsia, ofreciendo una conectividad talámica similar a la estimulación cerebral profunda (DBS). Esto respalda su uso para la focalización individualizada.

Principales métodos:
- Estudio monocéntrico (2018-2024) de pacientes con epilepsia con implantes de sEEG o DBS talámica.
- Se utilizaron sistemas estereotáxicos sin marco o robóticos para sEEG (electrodo de 0,8 mm) y marco rígido para DBS (cannula de 1,81 mm).
- Se evaluaron las complicaciones postoperatorias, la precisión de la focalización (proximidad a los núcleos del atlas de Morel) y la conectividad talámica mediante análisis del conectoma estructural.
Principales resultados:
- Se implantaron 160 electrodos de sEEG y 188 de DBS en 109 y 83 pacientes, respectivamente.
- Un paciente con sEEG (0,9%) tuvo una hemorragia sintomática; 8 pacientes con DBS tuvieron complicaciones transitorias.
- Excelente precisión de focalización para ambos (mediana sEEG 0,30 mm, DBS 0,23 mm), con sEEG mostrando más valores atípicos (4% frente a 0%).
- Patrones de conectividad talámica altamente consistentes entre las cohortes de sEEG y DBS (rho de Spearman=0,86-0,98).
- Se identificaron perfiles de conectividad distintos para los núcleos ANT, CM y PUL.
Conclusiones:
- La sEEG talámica exhibe un perfil de seguridad favorable y una excelente precisión de focalización en comparación con la DBS.
- El menor diámetro del electrodo de sEEG probablemente contribuye a su seguridad.
- Los perfiles de conectividad talámica consistentes y distintos respaldan las estrategias de focalización individualizada utilizando sEEG talámica.
