Resumen

La disfunción hepática afecta hasta al 40% de los pacientes de la UCI, lo que aumenta la mortalidad. La detección temprana a través de pruebas dinámicas de la función hepática y la comprensión de la insuficiencia hepática aguda sobre crónica (ACLF) son cruciales para mejores resultados.

Sus antecedentes:

  • La disfunción hepática es común en pacientes críticamente enfermos (20-40%) y se asocia con peores resultados.
  • Las pruebas de función hepática anormales (PFH) pueden deberse a sepsis, toxicidad por fármacos, hipoxia o enfermedad hepática preexistente.
  • La disfunción hepática puede ser una razón principal para el ingreso en la UCI o desarrollarse durante la hospitalización.

Objetivo del estudio:

  • Revisar los avances recientes en la comprensión de la disfunción hepática en pacientes críticamente enfermos.
  • Enfatizar el diagnóstico y manejo de la insuficiencia hepática aguda sobre crónica (ACLF).
  • Destacar la importancia de las pruebas dinámicas de la función hepática para la detección temprana y el pronóstico.

Principales métodos:

  • Revisión de la literatura reciente sobre disfunción hepática en enfermedades críticas.
  • Enfoque en pruebas dinámicas de la función hepática (por ejemplo, depuración de indocianina verde).
  • Exploración de conceptos en evolución en la insuficiencia hepática aguda sobre crónica (ACLF).

Principales resultados:

  • La prueba dinámica de la función hepática ayuda en la detección temprana y el pronóstico.
