Resumen

La implementación de nuevos criterios de gestión de consultas redujo significativamente los tiempos de espera para las evaluaciones de demencia en una clínica de memoria de VA. Este proyecto de mejora de la calidad optimizó la atención al paciente, reduciendo las listas de espera de seis meses a menos de un mes.

Sus antecedentes:

  • Los neuropsicólogos y los equipos de evaluación de demencia en la Administración de Atención Médica de Veteranos (VA) experimentan importantes retrasos de pacientes, exacerbados por la pandemia de COVID-19.
  • Las largas listas de espera pueden provocar retrasos en la atención al paciente, lo que requiere estrategias efectivas para la reducción de listas de espera.

Objetivo del estudio:

  • Mejorar la eficiencia clínica dentro de una clínica interdisciplinaria de trastornos de la memoria en el sistema de atención médica de VA del centro de Arkansas.
  • Implementar criterios integrales para optimizar la gestión de consultas y reducir las listas de espera de pacientes.

Principales métodos:

  • Se utilizó el marco de Alcance, Efectividad, Adopción, Implementación y Mantenimiento (RE-AIM) y el Modelo de Implementación y Sostenibilidad Práctico y Robusto (PRISM).
  • Se desarrollaron y aplicaron criterios de gestión de consultas a través de revisiones de historias clínicas para triar referencias de manera efectiva.
  • Se incorporó un mayor apoyo administrativo y educación para los proveedores.

Principales resultados:

  • Se revisaron 195 consultas entre agosto de 2023 y abril de 2024.
  • Aproximadamente el 40% de las referencias se triaron con éxito a servicios alternativos.
  • Se redujo la lista de espera de la clínica de aproximadamente 6 meses a menos de 1 mes, con resultados sostenidos.

Conclusiones:

  • La implementación de criterios de gestión de consultas mejoró significativamente la eficiencia de la clínica.
  • La priorización de pacientes y las referencias alternativas optimizaron la prestación de atención.
  • El proyecto de Mejora de la Calidad redujo con éxito las listas de espera y mejoró el acceso de los pacientes a evaluaciones oportunas de demencia.
