Sus antecedentes:

  • Ohio enfrenta altas tasas de morbilidad y mortalidad por enfermedades cardiovasculares (ECV).
  • Adopción limitada de intervenciones de manejo de factores de riesgo de ECV en entornos de atención primaria.
  • Las iniciativas estatales financiadas por la Agencia para la Investigación y la Calidad en Atención Médica tienen como objetivo mejorar la implementación de prácticas basadas en la evidencia.

Objetivo del estudio:

  • Evaluar la efectividad de la Iniciativa Heart Healthy Ohio (HHOI), un programa de mejora de la calidad (MC).
  • Evaluar el impacto de HHOI en la mejora del control de la presión arterial en pacientes adultos de atención primaria.
  • Analizar las medidas de proceso y los resultados secundarios como la cesación del tabaquismo.

Principales métodos:

  • Un estudio de MC pre-post, de corte transversal repetido utilizando datos del registro electrónico de salud (REH).
  • Involucró a 293.638 pacientes adultos de 48 clínicas de atención primaria en 21 condados de Ohio.
  • Proporcionó apoyo estructurado de MC, entrenamiento, aprendizaje entre pares y retroalimentación de datos para estrategias de manejo de la hipertensión.

Principales resultados:

  • El control de la presión arterial mejoró del 67,7% al 70,7% después de la intervención.
  • Se observaron mayores mejoras en el control de la presión arterial en pacientes rurales y sin seguro.
  • Las medidas de proceso (medición de la presión arterial, seguimiento, intensificación de la medicación) se asociaron significativamente con un mejor control de la presión arterial.

Conclusiones:

  • La Iniciativa Heart Healthy Ohio (HHOI) demuestra una infraestructura de MC estatal factible y exitosa para mejorar el control de la presión arterial.
  • El modelo HHOI ofrece ideas para estrategias escalables para abordar el control de la hipertensión.
  • El éxito de la iniciativa sugiere una posible replicabilidad en otros estados que enfrentan desafíos similares de ECV.
