La vasculitis urticarial hipocomplementémica (HUV) es una afección rara que a menudo se asocia con enfermedades autoinmunes como el lupus. El diagnóstico temprano y la atención multidisciplinaria son cruciales para el manejo de la HUV y la prevención de posibles complicaciones sistémicas.

Sus antecedentes:

  • La vasculitis urticarial (UV) se caracteriza por lesiones de pequeños vasos inflamatorios con ronchas que duran más de 24 horas.
  • La UV se clasifica en tipos normocomplementémico (NUV) e hipocomplementémico (HUV).
  • La vasculitis urticarial hipocomplementémica (HUV) es rara y frecuentemente se asocia con enfermedades autoinmunes, notablemente el lupus eritematoso sistémico (LES), lo que complica el diagnóstico.

Objetivo del estudio:

  • Presentar un caso de HUV en una paciente con antecedentes familiares de LES.
  • Destacar los desafíos diagnósticos y la importancia de un enfoque multidisciplinario en el manejo de la HUV.
  • Enfatizar la necesidad de una monitorización estrecha para el desarrollo potencial de LES en pacientes con HUV.

Principales métodos:

  • Presentación clínica de una mujer de 67 años con una historia de ocho meses de dermatosis diseminada.
  • Investigaciones de laboratorio que incluyen niveles de complemento sérico y pruebas de autoanticuerpos (ANA, anti-ADNds, anti-Sm, anti-Ro/SSA, anti-La/SSB).
  • Análisis de biopsia de piel para hallazgos histopatológicos (infiltrado perivascular, polvo nuclear, necrosis fibrinoide).

Principales resultados:

  • La histopatología reveló un infiltrado perivascular predominantemente neutrofílico con polvo nuclear y necrosis fibrinoide leve.
  • Los resultados de laboratorio mostraron niveles bajos de complemento sérico; sin embargo, los anticuerpos específicos asociados con LES fueron negativos.
  • La paciente respondió favorablemente al tratamiento con prednisona (0.5 mg/kg/día con reducción semanal) y monitorización estrecha.

Conclusiones:

  • A pesar de la negatividad de autoanticuerpos específicos, los hallazgos clínicos, de laboratorio e histopatológicos respaldaron un diagnóstico de HUV.
  • La identificación temprana y una estrategia multidisciplinaria son vitales para el manejo de la HUV.
  • El seguimiento vigilante es esencial debido al alto riesgo de desarrollar LES en pacientes con HUV.
