Este estudio presenta la separación de fuentes de susceptibilidad basada en tiempo de eco ultracorto (UTE) para imágenes musculoesqueléticas. Sus antecedentes:

  • Las imágenes musculoesqueléticas (MSK) a menudo enfrentan desafíos para diferenciar los componentes tisulares.
  • El mapeo cuantitativo de susceptibilidad (QSM) puede verse confundido por efectos mixtos de susceptibilidad magnética.
  • La artropatía hemofílica (AH) implica daño articular, con deposición de hemosiderina como característica patológica clave.

Objetivo del estudio:

  • Demostrar la viabilidad de la separación de fuentes de susceptibilidad basada en tiempo de eco ultracorto (UTE) para imágenes MSK.
  • Permitir la discriminación entre componentes tisulares diamagnéticos y paramagnéticos.
  • Mejorar la evaluación de la composición tisular en afecciones como la HA.

Principales métodos:

  • Integración de la descomposición iterativa de agua y grasa con estimación de asimetría de eco y mínimos cuadrados (IDEAL-QSM) para la estimación del campo B0.
  • Proyección sobre campos dipolares (PDF) para el mapeo de campo local.
  • Separación de Chi para mapeo cuantitativo de susceptibilidad (QSM) con descomposición de fuentes utilizando secuencias UTE.

Principales resultados:

  • Los estudios de fantomas mostraron que los mapas de separación de fuentes reflejaban con precisión las concentraciones diamagnéticas (CaCO3) y paramagnéticas (Fe3O4), a diferencia de la QSM convencional.
  • La QSM-UTE in vivo en pacientes con HA reveló una mejor visualización de los depósitos de hemosiderina paramagnética.
  • Los mapas paramagnéticos delinearon eficazmente la hemosiderina, superando los efectos de cancelación observados en la QSM convencional.

Conclusiones:

  • La separación de fuentes cuantitativa de susceptibilidad basada en UTE es factible para aplicaciones MSK.
  • Este método mejora la detección de sustancias paramagnéticas como la hemosiderina en la AH.
  • La técnica tiene potencial para una mejor evaluación de la composición del tejido óseo y articular.
