Resumen

BayesCNV detecta con precisión las variaciones del número de copias (CNV) en el ADN libre de células (cfDNA) utilizando un novedoso modelo bayesiano. Este método ofrece una mayor sensibilidad y especificidad para los paneles de secuenciación dirigida, mejorando la fiabilidad diagnóstica.

Sus antecedentes:

  • La detección de variaciones del número de copias (CNV) en datos de secuenciación de próxima generación (NGS) es un desafío, especialmente con el ADN libre de células (cfDNA) de baja señal y los paneles dirigidos.
  • Los altos niveles de ruido en la secuenciación de cfDNA complican la identificación precisa de CNV.

Objetivo del estudio:

  • Desarrollar y validar BayesCNV, un modelo jerárquico bayesiano para la estimación robusta de la relación de copias a nivel de gen a partir de datos de secuenciación dirigida.
  • Proporcionar una llamada de CNV precisa con cuantificación de la incertidumbre y una métrica de control de calidad (QC) basada en evidencia para el análisis de cfDNA.

Principales métodos:

  • Implementación de un modelo jerárquico bayesiano para la estimación de la relación de copias a nivel de gen utilizando profundidades de lectura de amplicones dirigidos.
  • Utilización de la integración termodinámica para la estimación fiable de la verosimilitud logarítmica marginal para el control de calidad.
  • Comparación de BayesCNV con IonCopy y DeviCNV utilizando muestras de referencia con CNV conocidas en el panel OncoReveal Core Lbx.

Principales resultados:

  • BayesCNV logró una sensibilidad de 0,87 y una especificidad de 0,996, superando a los métodos de la competencia.
  • La verosimilitud logarítmica marginal distinguió eficazmente las muestras degradadas de las de alta calidad en conjuntos de datos FFPE, superando las métricas convencionales de control de calidad.
  • Demostró estimaciones de CNV a nivel de gen precisas e interpretables con cuantificación de la incertidumbre.

Conclusiones:

  • BayesCNV ofrece una solución robusta y precisa para la detección de CNV en la secuenciación dirigida de cfDNA.
  • La métrica de control de calidad integrada mejora la fiabilidad de la llamada de CNV en muestras desafiantes de bajo aporte.
  • El método proporciona resultados interpretables y cuantificación de la incertidumbre cruciales para aplicaciones clínicas.
