Sus antecedentes:

  • La condición post-COVID (PCC) puede implicar inflamación persistente de bajo grado, afectando los resultados de las pruebas de laboratorio.
  • La utilidad diagnóstica y de seguimiento de las pruebas de laboratorio de rutina para la PCC sigue en investigación.

Objetivo del estudio:

  • Investigar las alteraciones de los marcadores bioquímicos en relación con distintos fenotipos de PCC hasta 18 meses después de la infección por SARS-CoV-2.
  • Evaluar la correlación entre la bioquímica de la infección aguda y el desarrollo posterior de PCC.
  • Evaluar el vínculo entre la gravedad de la infección aguda y las anomalías bioquímicas de seguimiento.

Principales métodos:

  • Estudio prospectivo de cohorte multinacional (ORCHESTRA) con 4587 pacientes con SARS-CoV-2 seguidos durante 18 meses.
  • Análisis multivariante de análisis de sangre recolectados durante la infección aguda y a los 6, 12 y 18 meses.
  • Identificación de cuatro fenotipos de PCC (RESc, CPc, CFc, NSc) mediante análisis de componentes principales.

Principales resultados:

  • Los pacientes con PCC, particularmente el fenotipo respiratorio (RESc), mostraron niveles significativamente más altos de proteína C reactiva (CRP) a los 12 y 18 meses en comparación con los controles.
  • Los niveles de CRP en pacientes con PCC y RESc superaron consistentemente los 3 mg/L, lo que indica inflamación de bajo grado.
  • La gravedad de la COVID-19 aguda se correlacionó con niveles elevados de CRP, ferritina y LDH durante el seguimiento; las anomalías bioquímicas tempranas no predijeron el desarrollo de PCC.

Conclusiones:

  • Las anomalías en los análisis de sangre consistentes con inflamación sostenida de bajo grado son detectables hasta 18 meses en el fenotipo RESc de PCC.
  • Estos hallazgos respaldan un papel para la inflamación prolongada en la patogénesis de la PCC.
  • Se recomiendan ensayos clínicos dirigidos a terapias antiinflamatorias e intervenciones específicas para los síntomas en pacientes con RESc.
