Resumen

Sus antecedentes:

  • Las hiperintensidades de la sustancia blanca (HSB) en la RMN se asocian con el deterioro cognitivo.
  • La calificación visual actual (escala de Fazekas) es subjetiva y carece de precisión.
  • Se necesita una cuantificación objetiva de las HSB para una mejor evaluación cognitiva.

Objetivo del estudio:

  • Comparar el análisis volumétrico automatizado de HSB con la calificación visual para la evaluación del deterioro cognitivo.
  • Evaluar la asociación entre el volumen, la distribución de las HSB y el estado cognitivo.
  • Determinar si la cuantificación automatizada es superior a la calificación visual en la correlación con el rendimiento cognitivo.

Principales métodos:

  • Análisis retrospectivo de 41 RMN de adultos con preocupaciones cognitivas.
  • Segmentación automatizada de HSB utilizando el software Icometrix para volúmenes totales y regionales.
  • Rendimiento cognitivo evaluado mediante la Montreal Cognitive Assessment (MoCA) y agrupado en alto, moderado y bajo.
  • Comparación de volúmenes automatizados y grados de la escala de Fazekas con las puntuaciones de la MoCA.

Principales resultados:

  • El mayor volumen total de HSB se correlacionó significativamente con puntuaciones más bajas de la MoCA.
  • El volumen automatizado de HSB mostró diferencias significativas entre los grupos cognitivos.
  • La escala de Fazekas demostró una correlación más débil con las puntuaciones de la MoCA en comparación con el volumen automatizado.
  • El aumento escalonado de la carga de HSB volumétrica en las categorías de Fazekas confirmó la validez del método.

Conclusiones:

  • La cuantificación volumétrica automatizada de HSB es más objetiva, sensible y escalable que la calificación visual.
  • El análisis automatizado proporciona una medida más precisa de la carga de HSB y su asociación con el estado cognitivo.
  • Este método es más adecuado para el seguimiento longitudinal y la investigación en el deterioro cognitivo.
