Sus antecedentes:

  • La Reparación de Lichtenstein (RL) es el estándar de oro mundial para la reparación de hernias inguinales debido a su simplicidad, reproducibilidad y idoneidad para la anestesia local.
  • La adopción mundial de la RL ha llevado a numerosas variaciones, con impactos poco claros en la recurrencia y el dolor crónico.

Objetivo del estudio:

  • Investigar las variaciones en la técnica de Reparación de Lichtenstein (RL).
  • Identificar los factores que contribuyen a la divergencia de los métodos de RL.
  • Evaluar la influencia de las modificaciones en los resultados de la reparación de hernias.

Principales métodos:

  • Revisión de la literatura de siete libros de texto importantes de cirugía.
  • Una encuesta de 17 preguntas distribuida a 90 cirujanos en 19 hospitales de Austria y Eslovenia.
  • Análisis de las respuestas de los cirujanos sobre los pasos críticos de la RL.

Principales resultados:

  • Existen variaciones significativas en las descripciones de los libros de texto y en la práctica quirúrgica de la RL.
  • Más del 50% de los cirujanos informaron recortar las mallas preformadas, lo que podría comprometer la cobertura inguinal.
  • Con frecuencia falta un seguimiento rutinario de los pacientes después de la Reparación de Lichtenstein.

Conclusiones:

  • El término "Reparación de Lichtenstein" ahora cubre ampliamente diversas técnicas de malla anterior.
  • La adherencia a los principios basados en la evidencia de la RL original es vital para minimizar la recurrencia y el dolor crónico.
  • La estandarización de la técnica de RL es esencial para obtener datos precisos y comparables en la investigación y los registros.
