Sus antecedentes:

  • La encefalitis por herpes simple (EHS) es una afección neurológica grave.
  • La EHS provoca un deterioro cognitivo significativo y cambios estructurales en el cerebro.
  • Comprender los mecanismos neurobiológicos subyacentes es crucial para un tratamiento eficaz.

Objetivo del estudio:

  • Investigar las alteraciones microestructurales y de conectividad funcional (FC) en pacientes con EHS.
  • Evaluar la asociación entre estos cambios cerebrales y la función cognitiva.
  • Explorar la relación con parámetros del líquido cefalorraquídeo (LCR) y marcadores inflamatorios.

Principales métodos:

  • Estudio de cohorte que incluyó a 73 pacientes con EHS y 76 controles.
  • Se utilizaron la morfometría basada en vóxeles (VBM) y la resonancia magnética funcional en estado de reposo (rs-fMRI).
  • Se midió la función cognitiva (MoCA), la presión del LCR, las proteínas y las citoquinas (IL-6).

Principales resultados:

  • Los pacientes con EHS mostraron una reducción del volumen de materia gris (VGM) en regiones cerebrales específicas.
  • Se observó una FC alterada entre el hipocampo, el precúneo, la circunvolución cingulada y la mPFC.
  • Las puntuaciones cognitivas más bajas se correlacionaron con cambios en el VGM/FC; la presión del LCR elevada, las proteínas y la IL-6 se vincularon con peores métricas cerebrales.

Conclusiones:

  • La EHS se asocia con cambios microestructurales y de FC distintivos que se correlacionan con el deterioro cognitivo y la neuroinflamación.
  • Se identificó una posible interacción entre la IL-6, las alteraciones cerebrales y la disfunción cognitiva.
  • Los hallazgos pueden guiar estrategias terapéuticas personalizadas utilizando biomarcadores de neuroimagen e inflamatorios.
